Alta Sierra Veterinary Hospital offers our patient form(s) online so you can complete it in the convenience of your own home or office.
Prescription Request Form
If blood work or exams are necessary before a refill can be authorized you will be contacted. Also allow 24 hours for us to refill your prescription. All special orders of medications must be placed Monday by 12 pm, otherwise medication will be ordered and available for the following week. This form will be automatically e-mailed to us.
Food Order Request Form
If you are unfamiliar with our food products please see our Store tab above to see all your options. You can easily copy your selection from that page and then paste it onto the Order Form. All food orders must be placed Monday by 12pm otherwise your food will be ordered the following week. You will be notified when your order comes in. This form will be automatically e-mailed to us.

This lets us record our pet and owners into our management software so that we can effectively and efficiently provide the service you deserve! You can print and bring this form already filled out in with you or fax it to us to save time.
Member Wellness Registration Form - Optional
This form can be filled out to register for access to the member wellness section of our website. You can also sign up for our monthly newsletter to keep up on current health issues and news and events in our office.
